The three surveys listed below are distributed by Next Shark, A3PCON, and ASHAAA. ASHAAA is not affiliated with Next Shark or A3PCON and to our knowledge, Next Shark and A3PCON are also two independent organizations. You are welcome to fill out as few or as many of the surveys as you would like.
These surveys are NOT legal complaints and will NOT lead to formal investigations. The purpose of these surveys is to capture the Asian and Asian American community’s experience with discrimination during COVID-19.

The A3PCON (Asian Pacific Policy Planning Council) provides incident reporting forms in various languages.
Incident Demographic Survey
ASHAAA is conducting a survey to further gauge how COVID-19 is affecting the Asian and Asian American community. In this survey, we ask you simple questions regarding your feelings during COVID-19 and, if exposed to discrimination incidents, how you react. This survey may lead to a follow-up project in which we conduct interviews with a few members of the Asian and Asian American community. The survey should take no longer than 5 minutes to complete and no identifying information will be collected. Please only submit answers for yourself, unless you have permission from another person to represent their responses. Otherwise, feel free to forward this survey to anyone whom you think may be interested in participating.
