Dear Mr. John Smith,

My name is Susan Brown. I am writing to file a discrimination complaint against:

Sullivan’s

100 Church Street

New York, NY 98765

(999)-888-1010

On March 30th, 2020, I believe I was subjected to discriminatory behavior at Sullivan’s in New York, NY. The checkout clerk named Samantha Clark at Sullivan’s refused to serve me because of my race.

Specifically, what happened was that Samantha snorted at me and muttered the racial slur “chink” under her breath. She said she would be right back and had to check on something, and disappeared into a back room. There were not a lot of clerks, so I waited for a few minutes, but when I saw the clerk Jake Lee become available, I approached him and asked him to help me find his, and hopefully Samantha’s, manager. During this time, Samantha did not come back out to the cashier registers. I told the manager Matthew Black what happened, and he apologized for the incident. I have not heard back from Sullivan’s in over a month on what action they are taking to address this issue.

I would like to inquire what the necessary next steps are to file a formal complaint against Sullivan’s. I am also wondering how long this process might take.

Attached is an audio clip of my conversation with Matthew Black and my receipt for my purchase that day at Sullivan’s, which I hope will be useful for looking deeper into this incident. For further information or to get in touch with me, you can reach me at:
